New Pet Intro Form - Required
All new clients & dogs must first complete:
- New pet intro form (Download Here)
- Provide shot records (your vet can email or fax them)
- Complete their interview (free trial day) to officially join the Lucky Dog family (not applicable to grooming only dogs)
Free tours (without your pup coming along) are available as well if you would like to check us out before going through with an interview and trial day.
For the safety of all the dogs, we have to verify that new dogs are healthy and up-to-date on their records before allowing them in the building.
To request your free trial day and interview, call us at 618-277-2275 or email us. You must also fill out the new pet intro form and provide vaccination records (your vet can email or fax them - Fax: 618-277-2276. Email: swansea@luckydogbarks.com).
Interviews are conducted Monday through Friday, by appointment only. We will spend 20-30 minutes with you, the owners, getting to know you and your dog(s). Your dog(s) will then stay with us for a minimum of 3 hours to make friends, have some time to explore, and settle in.
Your dog must meet the following requirements:
- Be at least 4-months-old
- Be spayed or neutered if 8-months or older
- Current on Rabies, Distemper, 6-month Bordetella, & Canine Flu (shot records are required)
- Healthy
- Flea/tick free
- Dog-friendly
- Generally love to play
- Pass an interview/socialization/temperament test (free trial day)
To book only grooming services, the new pet intro form is required. Please complete it and email it per the instructions. In the subject line, please specify that it's for Grooming Only. Once we have reviewed it, we will contact you to schedule your appointment.
Grooming only dogs are required to have the following vaccinations:
- Rabies
- Distemper
- 6-month Bordetella
