Lucky Dog Requirements, Policies & Procedures

Due to the nature of dog-play environments, all prospective clients are asked to read and review our following contract and initial each line when the pet owner has read, understood. and agreed to the information here within. Natural risks such as injury, disease, illness, death, property damage, and unforeseen incidents are possible. Dogs will play with one another with teeth and claws. Scratches, punctures, and hair loss are normal aftermaths of dog-to-dog activities. Over-exertion, exposure to heat and other natural elements, and diseases associated with physical activity are inherent. We also manage our play yards using noninvasive methods such as water spray bottles, time-outs, and temporary detainment (holding a collar) to help control our play environment and prevent altercations. You (the pet owner) give our staff and associates permission to use these proven methods on your dog at our discretion. Our boarding guests will be given one and a half hour breaks three times a day during meals and will be secured in their kennels overnight when not supervised. Dogs who are not eating may be given additives if allergies are not indicated by the owner. If you have questions, concerns, or comments please advise our management for clarification.

 ~Lucky Dogs Barks & Recreation

1. All dogs must:

  • Be current on vaccinations; Rabies, Distemper, Canine Influenza and Bordetella within 6 months

  • Be at least 4 months of age (Boarding & Daycare Clients)Be spayed or neutered if over 8 months of age (Boarding & Daycare Clients)

  • Be in general good health and free of parasites (must be on preventative care)Be non-aggressive towards other dogs and people

  • Pass the temperament test set by Lucky Dogs Barks & Recreation (Boarding & Daycare Clients)

  • Meet all municipal and state licenses and other requirements

  • Use a quick-release style collar and appropriate identification with dog’s name clearly displayed (Boarding & Daycare Clients)

  • Be leashed when entering or exiting Lucky Dogs Barks & Recreation on a 6-foot or shorter leash

2. Food (Boarding Clients)

Dog will be fed in accordance with the instructions provided in the application. Since feeding is sometimes adjusted based on your dog’s dietary needs, please provide us with written feeding instructions at drop-off

3. Dog Camera and Video Release

By using the services offered by Lucky Dogs Barks & Recreation you agree to allow us to use your dog’s name and any image or likeness of your dog(s) taken at Lucky Dogs Barks & Recreation in any form or format, at any time, in any media, marketing, advertising, illustration or promotional material.

4. Personal Property

Lucky Dogs Barks & Recreation will not be liable for any lost, stolen, or damaged personal property belonging either to you or your dog. If your dog causes any personal property damage to the facility where services are provided, another dog or otherwise, you will be liable for the full cost of all treatment, repairs, and replacements.

5. Fees

You must pay for all services and products with credit card, cash or check. All payment is due on or before the time of pick-up for services rendered. You give your expressed permission for Lucky Dogs Barks & Recreation to charge any credit card you provided for unpaid services or products upon the date services were to have been completed. You further agree to pay any cost of any returned/challenged checks or debit charges.

6. Deposits, Cancellations and Refund Policy

We require a deposit at the time the reservation is made for all boarding stays.  A (1) night deposit for all regular boarding stays and a (3) night deposit for boarding over MAJOR HOLIDAYS.  You will forfeit your deposit/charged (1) full night, if you fail to cancel your boarding reservation at least (48) hours in advanced of drop-off date.  If your boarding reservation is within (3) days of national recognized holiday you must cancel your reservation within (72) hours or you will forfeit your deposit or charged a “no-show” fee equal to three nights of boarding.  All Package sales are final, once started, and nonrefundable.  

7. Early drop off/ Late Pick-Up Fee

Dogs can be dropped off half an hour before the posted opening time for an additional $10. Late pickup-$20 from 7:05 until 7:20pm on weekdays, holidays, and weekend evenings, and 11:05am until 11:20am on weekend and holiday mornings. After 7:20pm/11:20am MANDATORY OVERNIGHT BOARDING.  This is at the closing employee’s discretion and for the safety of staff.  Boarding Guests will be charged a $15 daycare fee if checking out after 1pm on weekdays and 11am on weekends.  Late Check-Out fee is waived if your dog is scheduled for a Bath or Groom during their stay.

8. Matted Pets (Grooming Clients)

Pets with matted coats need extra attention during their grooming session. A “Dematting Fee'' ($15/30 Minutes) will apply to all matted pets. Mats left in a pet’s coat will only grow tighter. Heavy matting can also trap moisture near the pet's skin allowing mold, fungus, or bacteria to grow, causing skin irritations that existed prior to the grooming process. Mats can be very difficult to remove and may require the pet to be shaved. When necessary, removing a heavily matted coat includes risks of nicks, cuts, or abrasions due to warts, moles or skin folds trapped in the mats. After effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations, abrasions, and hematomas.  Hematomas  are a severe bruise caused by blood vessels bursting beneath the skin, causing a pocket of blood within the tissue.  Hematomas are commonly formed when severe matting is removed from the ears allowing blood flow to quickly return to the dog’s ears. Shaved pets are also prone to sunburn and should either have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases, pets may also exhibit brief behavioral changes. Lucky Dog does not wish to cause serious or undue stress to your pet and will not continually de-mat your pet for you. Prevention is the best defense against matting by scheduling regular grooming appointments.

9. Fleas & Ticks (Grooming Clients)

If fleas and/or ticks are found on your pet during the grooming process, treatment to rid the pet of fleas and/or ticks is mandatory, and an additional charge will apply. This is not only to rid your pet of the unwanted pests, but to prevent them from transferring to other pets in the salon, as well as to avoid an infestation within the salon. We suggest keeping your pet on a monthly flea & tick treatment like Nexgard, Bravecto or Frontline.

10. Accidents (Grooming Clients)

There is always the possibility an accident could occur. Grooming equipment is sharp, even though we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when a pet is wiggling or moving around. Every effort will be made to ensure your pet is groomed as safely as possible. Lucky Dog Barks & Recreation will not be liable for any accidents that occur during the grooming process.

12. Aggressive Dogs

Dogs cannot be aggressive and must pass Lucky Dogs Barks & Recreation’s temperament test. Your dog may be separated from other dogs or asked to not return to Lucky Dogs Barks & Recreation if he/she exhibits aggressive or other unacceptable behavior. Such determinations will be made at the sole discretion of Lucky Dogs Barks & Recreation. Owners MUST inform Lucky Dog Barks & Recreation if your pet(s) may bite, have bitten, or shows signs of aggression towards people, other pets or during specific grooming procedures. Muzzles may be used if necessary during grooming procedures. Muzzling will not harm your pet, and protects both the pet and the groomer. A “Handling Fee” may be applied in addition to the regular grooming charge for aggressive or difficult to groom pets. Lucky Dog Barks & Recreation reserves the right to refuse/stop services for such pet(s) at any time before or during the grooming process. This is for the pet’s safety, as well as for the groomer’s safety.

11. Health & Senior Pets (Grooming Clients)

Grooming procedures can sometimes be stressful, especially for a senior pet or pet with health problems and can expose hidden medical problems or aggravate a current one during or after the groom. Because senior pets and pets with health problems have a greater chance of injury, these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress. In the best interest of your pet this contract/agreement will give Lucky Dog Barks & Recreation permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary. We will do our best to contact you first. It is agreed that all expenses for Veterinary care will be covered by the pet's owner.

13. Duty to Disclose

You must disclose, on a continuing basis, all medical or other conditions, including but not limited to, personality concerns or behavioral incidents that in, Lucky Dogs Barks & Recreation’s sole discretion, may affect, limit, or prevent your dog’s ability to participate in Lucky Dogs Barks & Recreation activity or services.

14. Acceptance and Acknowledgement of Risk:

You fully understand that : (a) there are inherent and potential risks involved with interactions between humans and dogs, as well as between dogs and other dogs, which may result in property damage or bodily injury, including but not limited to, permanent disability, sickness, or death to human or dog; and (b) there may be other risks not known to you or Lucky Dogs Barks & Recreation, nor readily foreseeable at this time (collectively, “risks”). You fully accept and assume all risks and responsibility for all risks, without limitation, all losses, costs, and damages incurred as a result of your dog’s participation, including any veterinarian expenses incurred on behalf of or caused by your dog. “Sickness” includes any illness not limited to Bordetella (kennel cough), bloat, or any other form of contagious disease.

15. Abandoned Dogs:

No dog may be left after the scheduled pick-up time without communication from her or his owner. If your dog(s) is left without any contact, instruction, or notification regarding its pick-up from you or an authorized representative, your dog will be deemed abandoned at 7:00 pm on the 7th day after the scheduled pick-up date. If your dog is abandoned, Lucky Dogs Barks & Recreation will become its legal owner and guardian. Lucky Dogs Barks & Recreation will, at its sole discretion, determine whether to rehome or relinquish it to a shelter or rescue at Lucky Dogs Barks & Recreation’s choice. YOU FULLY UNDERSTAND AND AGREE THAT IF YOU ABANDON YOUR DOG AT Lucky Dogs Barks & Recreation YOU MAY BE UNABLE TO RETRIEVE POSSESSION OF YOUR DOG AND WILL HAVE NO RECOURSE AGAINST Lucky Dogs Barks & Recreation.

16. Veterinarian Liability and Care:

Lucky Dogs Barks & Recreation will have the right to obtain medical treatment for your dog if, in Lucky Dogs Barks & Recreation’s sole discretion, it appears that your dog may be ill, injured, or exhibits any other behavior that would reasonably suggest that your dog may need medical treatment. Lucky Dogs Barks & Recreation will first attempt to seek treatment from the veterinarian provided in the Dog Application but will have the right to seek veterinary treatment from an alternative veterinarian if Lucky Dogs Barks & Recreation is unable to reach or transport the dog to such veterinarian. YOU WILL BE FULLY RESPONSIBLE FOR ALL COSTS RELATED TO ANY MEDICAL TREATMENT, INCLUDING WITHOUT LIMITATION, THE COST OF ANY TRANSPORTATION FOR THE PURPOSE OF SUCH TREAMENT. You must also provide veterinary records to Lucky Dogs Barks & Recreation upon request.

17. Waiver, Release, and Indemnification:

You hereby agree to expressly and forever generally waive discharge claims, indemnify release from liability,save,hold harmless and defend Lucky Dogs Barks & Recreation, their invitees, owners, officers, directors, employees, contractors, volunteers, agents, representatives, lessors, and all others having an interest in Lucky Dogs Barks & Recreation (collectively, the “Releases”) from and against any and all injury, liability, claims, litigation, actions, suits, costs, losses, damages, attorney’s fees, expenses, or demands or every character whatsoever on account of, arising out of, resulting from or relating in any way to (i) any act or omission of the Releases, (ii) your dog’s participation at Lucky Dogs Barks & Recreation, services provided. YOU AGREE THAT THIS RELEASE IS BINDING ON YOU AND YOUR SUCCESSORS, HEIRS, LEGAL REPRESENATIVES, AND ASSIGNS. YOU ALSO EXPRESSIVELY AND FOREVER RELEASE Lucky Dogs Barks & Recreation FROM ANY DUTY TO PROTECT YOU OR YOUR DOG FROM INJURY OF ANY KIND AND AGREE THAT ANY IMPLEMENTATION OF SAFETY PRECAUTIONS BY Lucky Dogs Barks & Recreation WILL NOT WAIVE Lucky Dogs Barks & Recreation’s RIGHT TO BE INDEMNIFIED AS PROVIDED HEREUNDER AND SUCH PRECAUTIONS WILL NOT ALTER THIS RELEASE.

I acknowledge and agree that I have read, initialed, understood and agreed to all the terms and conditions contained in the dog release, waiver of liability, assumption of risk and indemnification agreement. I hereby execute this application and the agreement for my dog, myself and my heirs, successors, representatives, and assigns. I further attest that if I am not the owner or sole owner of the dog(s) for whom this application has been completed, my signature is sufficient to enter into this agreement for and on behalf of any other owner or representative. If signing electronically, I hereby agree that my signature will be deemed and original and take the place of my wet-ink signature. If signing in ink, I hereby agree that a true and correct copy of this document may be produced in lieu of the original application. Should a copy by produced, I understand that it is legally enforceable and does not affect the terms of the application in any way.
